To the employee
You have been directed to this page because your employer has sent you for a health check-up. Here you will find all the information on how to prepare for the health check and which documents need…
Four steps to your health check
-
1
Health condition
Please do not attend the health check while on sick leave – an acute illness may affect the results.
-
2
Online booking
Click "BOOK NOW!", select a location and service. Employees referred by their employer choose "Occupational health doctor's appointment (contracted client)". Clients without a contract choose "Occupational health doctor's appointment" and pay via bank link.

If the time no longer works
If you cannot attend, notify us as early as possible at info@mediserv.ee. If the visit is less than 48 hours away, call 5664 1211. Early notice lets us offer the slot to someone else in need.

Health declaration & ID
Complete the "Employee health declaration" and, if required, the "Mental health questionnaire" before your check. The declaration must not be shared with third parties. Bring an identity document (ID card/passport/driving licence).
Documents you need
Fill these in before your appointment and bring the completed forms with you.
Preparing for the tests
Depending on your health check package, the following points are important for you.
Blood sample
If you have been prescribed medication, take it as usual. Avoid eating or drinking for 2 hours before your appointment. The check usually includes a painless finger-prick blood sample, as we use special automatic needles.
Eye examination
If you wear glasses, bring them and, if possible, your current prescription. Railway workers, police officers and security guards should remove contact lenses 2 hours beforehand.
Urine sample
At least 4–6 hours should have passed since your last urination. The day before and until collection, avoid acidic foods, drinks and beetroot. Use the cup provided on-site, collect the first 20 ml (half the cup), close it tightly and write your name on it.
Cycle-ergometer stress test
A cardiovascular test evaluating your endurance. After a serious illness, come only once fully recovered. Wear comfortable sporty clothing and footwear and bring something sweet. Avoid strenuous activity for 24 h before. The maximum weight allowed is 160 kg.
Thank you for your understanding and cooperation!
