Before booking an appointment with the occupational health doctor, please read the instructions below:
- While on sick leave, please refrain from booking an appointment.
- The health check includes a finger prick blood test. Please do not eat or drink 2 hours before the appointment. If you wear glasses, please bring them along.
- Please bring an identification document (ID card/passport/driving license).
- Before your appointment, please fill out the health declaration:
